The number of adjunct therapy (Rx) added to a medium (MED) or high dose inhaled corticosteroids (ICS), is listed. Treatment steps were classified in accordance with the Global Initiative for Asthma (GINA); however, in children less than 6 years in whom there is no GINA Step 5, we assumed that those on Step 4, in whom tiotropium was an added adjunct therapy, have moved to Step 5
